In order to maintain a safe and healthy environment it is mandatory that the guidelines below are followed. PLEASE READ THROUGH ALL OF THE POLICIES.
AGE TO BOOK
For legal purposes you must be 18 or older to book. Parents or guardians may book for you. You will be required to submit a photo ID to verify your name, age and address.
SICK
If you are feeling under the weather at all (e.g. cough, fever, shortness of breath, etc.) please do not make an appointment. If I notice any of these symptoms you will be turned away and your retainer will not be refunded.
GUEST

RETAINERS
Retainers are non-refundable but are transferable.
